Please complete one form per student. Note that classes are held Mondays through Wednesdays from 6:30 pm to 8:00 pm. Please indicate the class you want on the form. All classes are 6 weeks and are $405.00.

This registration form will enroll you in the class. To finalize this process please pay online or send a check in the mail. When you submit this form you will be signing it electronically.

Terms of Use

Class Cancellation Policy: A session reservation may be cancelled up to one week before the session (viz. before May 15, 2017 for Session 1, before July 3, 2017 for Session 2, and before August 14, 2017 for Session 3). A $50.00 cancellation fee will be deducted and the remaining balance will be refunded. An e-mail cancellation notice must be sent to our website before the cancellation dates, and no refunds will be given after the cancellation dates. You may use our website’s contact form, too, to cancel the session participation.
If we cancel a session for any reason, a full refund will be given, or if requested applied to a future session.
